The Chapleau and District Family Health Team (CDFHT) is an interdisciplinary team of health care providers including Nurse Practitioners, Registered Nurses, a Pharmacist, a Respiratory Therapist and a team of Chiropodists. All of our providers work side-by-side with our local Family Physicians in the Chapleau Medical Centre to offer you access to the right service by the right provider at the right time.
