At North West Community Care Access Centre (North West CCAC), it is our drive to deliver exceptional home and community health care for people of all ages. We service Thunder Bay, Rainy River, and the Kenora district with health care support at home, at school, and in the community, as well as supported living and long-term care service.

At North West Community Care Access, our care coordinators are regulated health professionals with backgrounds in nursing, social work, occupational therapy, physiotherapy or speech therapy. They’ll work with you to develop the care plan you need, from nursing care and meal delivery, to help finding a doctor or a day program that will benefit you.
